You exceeded the maximum allowed number of login attempts

Today I received the following message when logging in:

You exceeded the maximum allowed number of login attempts. In addition to your username and password you now also have to enter the confirm code from the image you see below.

This is the second time this has happened, does anyone know why? Am I alone in receiving this message? Should I be concerned?
